Every step of a Re:Link placement, from referral to future pathway, in the open. Referrals are open to local authorities, social workers, schools and families.

We review EHCP outcomes and professional reports - Educational Psychology, Occupational Therapy, Speech and Language, behaviour and risk information, and previous educational placements.
The panel reviews suitability of provision, group compatibility, staffing requirements, risk management and therapeutic support requirements.
We complete risk assessments, gather medical information, complete “About Me” documentation, and build positive relationships with the child and family.
Building trust, developing relationships, creating emotional safety, understanding routines, communication preferences and supporting regulation.
Personalised provision begins - therapeutic education, sensory-based learning, community experiences, regulation support, life skills and reintegration activities.
Daily observations, session photographs, half-termly Nurtured Hub snapshots, EHCP outcome reviews, Child Development Meetings and family and professional feedback.
An End of Provision Assessment brings together development graphs and an EHCP outcome review - planning next steps in partnership with family, school and local authority.
